Issue:

Va employee handed an evidence file to the employee being charged. The evidence file was not properly redacted. A VA Form 0724 with a list of 20 names of employees along with their last four was contained in the file along with sworn statements and Uniform Offense Rreports. One of the sworn statements was not redacted to remove a testifying employee's full SSN. The employee received the file on Friday, August 3, 2012 and immediately brought it to the Union President. It was secured in the Union President's Office until given to the PO this morning by the Union VP. Update: 08/06/12:The one employee whose full SSN was disclosed will be offered credit protection services. The other 19 will be sent general notification letters.08/08/12:Several more employees were involved in this incident. The totals are now 4 for credit protection and 19 for general notification.

Outcome:

Responsible employee's supervisor notified. Employee counseled by PO and required to re complete Privacy and Security Training.
