Issue:

On June 11, 2012 the wife of Veteran A contacted the Privacy Officer (Nashville Campus) to report the following: Veteran A received a copy of his medical records, in the mail, from Release of Information (ROI), Nashville Campus. What the Veteran received also included medical records belonging to Veteran B (approximately 200 pages). Veteran A's wife stated Veteran B\xe2\x80\x99s personal identifiable information includes: medical records which contact the full name, full social security number, DOB, and address. Veteran A has an appointment at the Nashville Campus on Wednesday, June 13, 2012. The Privacy Officer made arrangements for Veteran A to return Veteran B\xe2\x80\x99s records to the Supervisor, ROI on this day. Privacy Officer reviewed the ROI package and noted the request to Veteran B also included 7332 (sensitive) information. Update: 06/11/12: Veteran B will be sent a letter offering credit protection services.

Outcome:

PO's have confirmed receipt of mismailed records by the Supervisor, ROI. The employee involved with this incident is no longer employed at the VA, so no action can be taken. ROI Supervisor will continue to educate ROI Staff on the importance of checking any mailings to insure no other records have been included or intermingled. CM Letter to be mailed this date and redacted copy uploaded.
