Issue:

Both Veteran A and Veteran B came in for Lab draws on 07/05/12. Veteran A requested a list of his appointments. Veteran A was given a copy of Veteran B's appointments. Veteran A never realized that he had the wrong list until he tried to check in at Veterans B appointment date and time. The Medical Support Assistant (MSA) at that time figured out that Veteran A had Veteran B's appointment list. Update: 07/12/12:Veteran B will be sent a letter offering credit protection services.

Outcome:

The Supervisor at the CBOC reeducated all staff at the Staff meeting about being more careful with patient information
