Issue:
Copies of a Veteran's medical record were mailed to the wrong person. The person who received them knew one of the authors of a note and contacted her. The staff member then retrieved the records from the wrong recipient and brought them back to the VA to secure. Update: 02/04/13:The Veteran whose information was sent in error will be sent a letter offering credit protection services as his full SSN and medical information were disclosed.
