Issue:

An employee emailed Veterans/patients and included their name and phone number The employee also emailed the messages to himself and another patient. One email to himself contained patient name and last four SSN as well as medications and treatment note. Update: 02/17/12:The name and phone number of one Veteran and a statement that he was "a VA patient and 100 percent disabled vet" was sent to a minimum of three patients and the name and phone number of a minimum of three patients was sent to another Veteran. Letters of notification will be sent to four Veterans.

Outcome:

Findings and supporting documentation reviewed with Service Chief for proper Administrative Action. Outlook access revoked.
