Veteran reported to his provider that he received lab results for 3 other patients in the mail along with his information. The lab reports contain the Veterans names, mailing address, and biopsy results. The Veteran has returned the information to the medical center. SSN not involved. Update: 07/06/12:The three Veterans will be sent notification letters.

Documents returned to the medical center. Incident addressed by supervisor during staff meeting.
