Issue:

On 9/21/11, Veteran A requested a list of his future appointments with the Columbia, SC Dorn VA Medical Center. After the Veteran returned home that evening, the Veterans daughter (Dorn VA employee) reviewed the list of appointments and was surprised to see that Veteran Bs full name and full social security number were listed on the appointment list intended for Veteran A. Veteran As Urology appointment was inadvertently scheduled using the wrong Veterans name and social security number. Both Veterans have the same last four numbers of the social security, first and last name. The Privacy Officer was not notified of the incident on 9/21/11. Update: 09/28/11:Veteran B will be sent a letter offering credit protection services.

Outcome:

Staff is being re-educated on the need to safeguard patient information.
