Issue:

Veteran A called the Privacy Officer (PO) today stating the the Release of Information (ROI) section mailed records to his residence in August, 2011. Veteran A never received those records. Veteran A stated he thought the VAMC was behind and only contacted hospital because he needed the copies for a meeting today. Update: 03/15/12:There was a lost Personal Identity Verification (PIV) Card. This is a policy violation and no data breach has occurred.

Outcome:

Records were mailed via UPS from the facility to Veteran's home address. Veteran waits seven months to notify VAMC he never received them. UPS tracking cannot locate without the number and the VAMC does not retain numbers that far back. PO not sure if UPS made the mistake of delivering to wrong address or if Veteran did receive the records but needed a second copy and did not want to pay the fee. PO cannot determine who is at fault.
