Based on staff interview and record review, the facility failed to maintain the privacy and confidentiality for one person (Patient A) when a staff member accidentally sent Patient A's medical information to the wrong recipient.Findings:An interview was conducted on October 5,2012 with the facility's Privacy Officer (PO). The PO stated that on September 24, 2012 she was notified of a breach incident. The PO stated that on September 24, 2012 a staff member accidentally faxed medical information for Patient A to the wrong recipient. The PO stated that the breach incident was identified when the recipient called the facility to notify them that Patient A was not their patient. The PO stated that the recipient promptly returned the documents.A review on October 5, 2012 of the personal health information (PHI) disclosed in error included:Patient nameDate of birthMedical record numberLaboratory resultsPatient progress noteThe PO further stated that Patient A was notified of the incident.
