Issue:

The driver of a Disabled American Veterans transportation shuttle operated out of the Portland VA Medical Center drove off after a pick-up stop at 1PM today with the clip board containing his Veteran transport list on top of the vehicle. When it was noticed missing the driver turned around and retraced his route. The clipboard was found with all of its paperwork in a roadside ditch at 2:45 PM. The list contained the full names, home addresses, phone number and clinic appointment time for eight Veterans. No health information was included. The driver will be turning in the contents of the clipboard to the Privacy Officer upon his return to the facility. Update: 12/14/12:Letters of notification will be sent to eight Veterans.

Outcome:

The documents were recovered by the shuttle driver.
