Issue:
Veteran A contacted the Clinic Administrator of the VA Maury County CBOC, to notify them that when he received a Patient Notification Letter in the mail, he also received a Patient Notification Letter belonging to Veteran B. The Patient Notification Letter of Veteran B contained their Full name, Address and information concerning instructions on decreasing a certain type of medication. Veteran A has already mailed the document belonging to Veteran B to the VA Maury County CBOC, who then provided the document/letter to the Privacy Officer.
Outcome:
02/19/14: The Incident Resolution Team has determined that Veteran B will be sent a HIPAA notification letter due to Protected Health Information (PHI) being disclosed.
