Issue:

The ISO received phone call from Mandalay Bay Casino employee who stated she received a document by mistake, and agreed to destroy the document. Upon investigation the Pharmacist faxed the document to a nursing home regarding renewal of medication for Veteran; by using the fax speed dial. The number was checked and determined to be the correct number. Facility was having trouble during that timeframe with fax lines. PII was name and medication name only. Update: 09/12/12:The Veteran will be sent a notification letter due to name and PII being exposed.

Outcome:

The Pharmacist faxed the medication order request to the Nevada Veterans Nursing Home using the speed dial function on the fax machine. The fax was received by the Mandalay Bay Casino by mistake, who upon receiving the misdirected fax notified VA; and agreed to destroy the document. The Pharmacy Department checked the pre-programmed fax number on the speed dial and discovered it was the correct number to the Nevada Veterans Nursing Home. During the time period of the misdirected fax, the VA was having trouble with the fax lines throughout the facility, therefore it is concluded the fax line called the wrong number due to problems with the lines. No fault of the Pharmacist. Notification letter mailed and uploaded on September 12, 2012.
