Issue:
Medical records were faxed to the wrong number. The office who received the records called to notify this facility. The office stated they would shred the records. The individual who took the call failed to get any contact information from the caller. The caller said that they didn't have a patient by that name so it is assumed that it was a medical office. There were two fax numbers on the request. The facility will try to get in contact using the numbers. Update: 05/03/12:One Veteran will be sent a letter offering credit protection services.
