Issue:

On 1/4/2011 Veteran #1 received a reminder by mail of an upcoming Mental Health appointment. The Veteran realized that a second Mental Health appointment letter which was intended for another Veteran was attached to his letter. The Greenville SC CBOC discovered the error when Veteran #1contacted the clinic to notify the administrative clerk of the incident. The Greenville CBOC Supervisor was notified and a corrected MH Clinic appointment letter was mailed to Veteran #2. The information disclosed: Full name, last four digits of the social security number, address, the name and location of the MH Clinic. When: The date of the mis-mailing could not be determined. Update: 01/18/11:Veteran 2 will receive a letter of notification.

Outcome:

Employees were educated on the need to safeguard patient information.
