Issue:

A Veteran left a copy of his medical record in the lobby. Release of Information staff collected it and called him to pick it up. Update: 12/23/11:Records were left by the Veteran himself. This is not the fault of VA.12/28/11:Further conversation with hospital staff revealed that the records were left by a physician and not the Veteran. The Veteran will be sent a letter offering credit protection services.

Outcome:

Patient stated that he never left these records in our lobby. I show that we faxed them to the base hospital. I am working with the base HIPAA staff to see if any of their staff would have been in our facility with these documents because they were hole punched at the top the way that hospital hole punches and we don't hole punch the way they were found and our ROI staff are not even in the same building as where the records were found. 1-17-2012Called Offutt AFB Release of Information Supervisor. He investigated on his end and couldn't find anyone that would have brought these records to the VA and left them in the lobby. I asked him if he could tell me if they ever gave the Veteran a copy of what we faxed them and he said they don't track that. Without any video or any other information I have nothing to go off of to tell them who left the records in our lobby and am asking that this be closed as I am providing credit monitoring.
