Issue:

Based on record review and interview, the facility failed to ensure Patient A's Protected Healthcare Information (PHI) was not given to another patient, when staff did not follow the facility's discharge process, which resulted with Patient B receiving another patient's PHI without written authorization.Findings:During a record review on October 10, 2012, Patient A was 69 year old female admitted to emergency department (ED) on September 24, 2012, for evaluation and treatment. Patient 2 was a 62 year old male admitted to the facility's ED for Gastroenteritis. Upon discharge, Patient 2 was sent home with Patient 1's discharge instructions and prescription, but not with his much needed discharge instructions and prescription.During an interview with RN 1 on October 9, 2012, at 2:25 p.m., he stated, "The physician's placed the discharge instructions on the charts. The process is that the name of the patient is circled when the patient repeats their name the form is initialed and given to the patient. Patient 2 was in a rush. So instead of following the process, I placed the discharge instructions in his back pack." During an interview with compliance officer 1 (CO 1) on October 8, 2012, she stated, "The doctor realized the wrong information was given right away. Patient 2 discovered it a few days later and brought it back."
