Issue:
Based on staff interview and document review the Facility failed to prevent an unauthorized disclosure (Breach) of patient medical information (PHI) for 1 of 1 sampled patients (Patient 1). This failure had the potential to compromise the personal information for Patient 1.Findings:On 10/27/14 the Facility reported to the Department that Patient 1's discharge paperwork was included in the discharge paperwork of another patient. The breach of PHI was detected on 10/21/14 when the other patient returned the paperwork to the Facility.On 7/15/14, the Director, Risk Management was interviewed. The Director, Risk Management confirmed that the above breach occurred as reported.
