Issue:
Today, 03/27/15, the Privacy Officer (PO) received a report that a surgery schedule was found outside the facility on the sidewalk near a patient drop off-pick up area. The surgery schedule was dated 03/26/15, printed at 6:36 AM and was recovered by a manager at 4:10 PM yesterday. The two (2) page surgery schedule consisted of nine Veterans full names, full Social Security Numbers, case number, medical/surgical diagnoses, and procedures with surgeon. Additional notification and investigation to ensue.
Outcome:
04/30/15: The Incident Resolution Service Team has determined that nine Veterans will be sent letters offering credit protection services.
