Veteran (A) received a letter that contained another letter belonging to Veteran (B).

Outcome:

09/17/14: PO update -- St. Paul has investigated this case extensively and spoke with the Veteran. The Veteran received an envelope with his original request for information along with a copy of someone elses request for information. This packet originated from the VA Medical Center in Alexandria, LA. There is no record that St. Paul received this document from the Veteran, as it was not associated with any end product. Veteran reported that the information he received in error was turned over to his Supervisor in the New Orleans VA Regional Office who immediately forwarded the document to Privacy Officer at VAMC in Alexandria, Louisiana. St. Paul requests the ticket opened by our station be transferred for action by the VA Medical Center in Alexandria Louisiana. 09/19/14: Alexandria PO update -- Letter that was received by Veteran A was from an attorney representing Veteran B. PO received a copy of the 2-page letter in question from Veteran A - it was attached to several comp-related documents that would have come only from VBA. The letter was a request addressed to Alexandria VAMC for medical records and contained only the first name, last name and last four digits of SSN. PO was unable to locate in CPRS...so no address of Veteran is available unless attorney will provide to PO. This incident was addressed several weeks ago by Alexandria. Alexandria PO contacted attorney's office on 09/05/14 @11:14 a.m. to request address of Veteran, since Veteran is not in local CPRS. I was assured by attorney's assistant that this would be forthcoming, but to date Veteran address has not been provided. Followed-up with attorney assistant and was provided Veteran's address in OH, and the correct spelling of Veteran's name. Please authorize CML. 09/22/14: The Incident Resolution Service Team has determined that Veteran B will be sent a letter offering credit protection services.
