On Wednesday, April 27, 2011, A Puget Sound police officer lost his issued pocket size notebook containing private patient Personally Identifiable Information (PII) as well as officer notes for police reports. No other information is available at this time. Update: 04/28/11:This is the police notebook that all police carry to collect information at the scene of any incident or issue, they ask for the name, ssn, and dob of individuals involved and put that down. From that notebook, when back at the desk, they create their police reports.Eleven Veterans will be sent a letter offering credit protection services.
