Issue:
A Resident left her patient list in the Canteen. She put the list on the counter when she was paying for her food and forgot it. As soon as she realized it was missing, she went back but by then the list was turned over to the Volunteer Service and the Volunteer Service turned it over to the Chief of Staff Office. The list was in the Canteen no more than 30 minutes. The list contained the names, last four digits of the SSN and diagnoses for 15 patients. Update: 05/06/11:The 15 patients will receive a letter of notification.

The employee was counseled by their supervisor.
