Issue:

On 07/17/13, the Administrative Officer (AO) at a Community Based Outpatient Clinic (CBOC) reported to the Privacy Officer (PO) that Veteran B received a letter by mail intended for Veteran A. The letter was mailed by the CBOC to Veteran A on 06/24/13. Upon receipt, Veteran B mailed the letter back to the main facility who forwarded the letter to Veteran As provider. The letter contains Veteran As full name, full SSN, home address, diagnosis, CAT scan results, medical record results, with location and date of next appointment. Veteran B writes on the letter that he is concerned about the SSN exposure as he is expecting a reimbursement check from VA with hopes that his SSN is not exposed also. Additional notification, thank you letter, investigation, and corrective actions are pending.

Outcome:

07/19/13: Veteran A will receive a letter offering credit protection services.
