Veteran received a copy of his medical record. Within the medical records he received was an ECG report that had another Veteran's name and full ssn at the top of the page. The Veteran returned the ECG to the facility. Update: 01/30/12:Veteran A will be sent a letter offering credit protection services.

Report corrected removing incorrect information.
