Issue:

Today, 09/17/14, the Privacy Officer (PO) received a report from a Veteran through the Patient Advocate Office. Patient was on the inpatient Mental Health Unit. Had been recently aware that his medical records have been confused with that of his father, who has the same name. Due to this error, his father has received medical records and became familiar with his personal medical history and the fact that he is __________ (sensitive issue). This has caused a rift between himself and his family. His father's health has suffered. Veteran was put out of the family home. Not sure when his records were mixed into his fathers. Additional notification and investigation, to ensue.

Outcome:

09/17/14: The Incident Resolution Service Team determined that the Veteran will receive a HIPAA letter of notification.
