Issue:

Veteran A reported via telephone that he received an appointment letter pertaining to Veteran B. The letter contained Veteran B's full name and last four of the SSN. The PO was notified and contacted Veteran A to recover the document. Veteran A indicated that he has received information pertaining to other Veterans on two other occassions. Veteran A will relinquish all information to PO on his appointment date of 02/11/11. Veteran A lives four hours away from the facility. Update: 02/03/11:The PO is waiting for Veteran A to return all paperwork.02/07/11:According to the PO, Veteran A advised they have three other Veterans information. Therefore three (3) Veterans will receive a notification letter.02/15/11: A further update from the PO states that one letter involved the full name and partial SSN (Veteran B), one letter involved the full name and full SSN and PHI (Veteran C), the third letter only Name and appointment date/time (Veteran D). Therefore, Veteran B will receive a notification letter and Veteren C will receive a letter offering credit protection services.

Outcome:

Involved employee to receive additional training regarding privacy.Notification and credit monitoring letter forwarded to involved Veterans.Redacted copies of letters submitted to IRTC for closure.
