Issue:
A VA clerk was faxing information to the Social Security Administration (SSA). He verified the fax number with the SSA but, when he faxed the information, it went to a utility company. At this time, the Privacy Officer (PO) is unsure of the exact information that was sent. The clerk has been in touch with the recipient but the documents have already been disposed of. The information that was compromised included the Veteran's name, address, date of birth, full SSN and medical information regarding the results of an audiology examination. Update: 12/15/11:Veteran A will be sent a letter offering credit protection services.
Outcome:
Employee stated he had verified the number with SSA; reminded to verify & use caution when dialing FAX number
