Based on staff interview and record review, the facility failed to maintain the privacy and confidentiality for one patient (Patient A) when an employee inadvertently found Patient A's medical information in another patients medical file.Findings:An interview was conducted on 5/24/12 at 1:00 PM, with the facility's Privacy Officer. The PO stated that on 5/9/12, it was brought to her attention that a possible breach had occurred. The PO stated that on 5/9/12 an employee was pulling another patients medical file for insurance auditing and noticed paperwork for Patient A in the other patient's file.A review on 5/24/12 at 1:10 PM of the information disclosed in error contained the following personal health information (PHI)Patient nameAmbulance recordDate/time of serviceAgeSexVital signsThe PO stated that the insurance company was contacted to retrieve the information sent along with the other patient's file. The PO stated that Patient A was notified of the breach.
