A Release of Information (ROI) clerk mismailed Veteran B's medical records to Veteran A. Veteran A returned the records to the facility. Portions of the record were in paper format, others were on disc.
Outcome:
11/06/14: The Incident Resolution Service Team has determined that Veteran B will be sent a letter offering credit protection services.
