Veteran A received Veteran B's discharge instructions. Veteran B's name, full SSN, date of birth and medication information was compromised.

Outcome:

01/21/14: Veteran A called the Privacy Officer (PO) to report that he had received Veteran B's discharge instructions instead of his own when he was discharged from the ED on Friday. He was already home by the time he discovered Veteran B's instructions. He graciously agreed to mail the documents back to the PO. They probably will arrive late this afternoon or tomorrow as Monday was a holiday. 01/23/14: Veteran A sent the discharge instructions back to PO via USPS. Fact finding and remediation still ongoing by Chief Outpatient Service. 01/28/14: Veteran B will receive a letter offering credit protection services.
