Issue:

Based on interview and record review, the facility failed to protect Patient B's protected health information (PHI) from unauthorized disclosure. Patient A's was provided with Patient B's arm band and registration paperwork.. This resulted in the potential for misuse of Patient B's PHI. Findings:On August 10, 2011, at 2 p.m., the Information Privacy Officer (IPO) was interviewed. The IPO stated Patient A had Patient B's arm band on. Employee 1 had put the wrong armband on the patient and this was discovered by another employee.A review of the Patient B's information that was disclosed was conducted. Patient B's PHI reflected the patient's first and last name, date of birth and medical record number. The facility policy and procedure titled "Patient Identification Policy and Procedure (Last reviewed/Revised 01/03/2011), was reviewed. The policy indicated, "Scope and Applicability:..Procedure:..3. Patient registration...will verify the accuracy of the information on the identification bracelet with the patient or responsible party. 4. Patient registration or designated person will apply and secure the identification bracelet on the patient, confirming the correct patient name and patient birth date at the time of the encounter."
