Issue:
Patient A was given discharge paperwork that belonged to Patient B. Patient A presented the paperwork to the Pharmacy employee. The Pharmacist discovered the error and kept the paperwork. The paperwork contained Patient B's name, full SSN, date of birth and protected health information. Update: 04/22/11:Patient B will be sent a letter offering credit protection services.
