Issue:
Patient A was given paperwork on Patient B with the same last name. Patient A immediately noticed the discrepancy and reported back to Emergency Department to alert staff of the error. Patient A returned the incorrect paperwork, Staff apologized, and provided appropriate paperwork to the Patient A.
Outcome:
05/13/15: The Incident Resolution Service Team has determined that Patient B will be sent a letter offering credit protection services.
