Issue:
Two patients contacted our CBOC to notify us that one of them had received a form that belonged to the other. We cannot be certain of the details as we are only told the form was only partially filled in but it usually contains at least the full SSN, full name, DOB and financial information. The patient who received the document contacted the patient that it actually belonged to and then they each contacted our CBOC to try to find out how this error occurred. Update: 04/24/13:Two (2) Veterans will receive letters offering credit protection services due to full name and full SSN being disclosed.05/10/13:Update - only one Veteran's information was disclosed to another, therefore only one (1) Veteran will receive a letter offering credit protection services.05/22/13:This was determined to be HITECH reportable by VHA Privacy Office.

Employees reminded to use caution when handling documents with PII.
