Issue:
Patient A presented to the Birmingham VA Medical Center (BVAMC) Emergency Department on August 4, 2014 and was subsequently admitted and discharged on August 6, 2014. On August 7, 2014, Patient A presented to the BVAMC Emergency Department again and staff discovered Patient A's driver's license did not match the name he presented under. Staff notified the Business office of the issue and a new humanitarian patient account was created for Patient A. After that date, Patient B called the BVAMC Pharmacy department stating that he discovered medications were on his profile through MyHealtheVet that do not belong to him. It is the understanding of the BVAMC Privacy Officer that the medical records of both patients have been corrected. However, the discharge documents given to Patient A (humanitarian) contained PHI of Patient B (Veteran)although the exact information contained on the documents has yet to be determined.
Outcome:
09/12/14: The Incident Resolution Service Team has determined that Veteran B will be sent a letter offering credit protection services.
