Issue:

Based on interview and record review, the facility failed to prevent unauthorized access to a patient's (Patient A) protected health information (PHI) when Patient A's addressograph (a plastic blue card with the patient's information embossed on it) was used to imprint on another patient's discharge documents.Findings:On May 13, 2013, Patient A and Patient B were both being treated in the facility's emergency department. When discharging Patient B, the Registered Nurse used Patient A's addressograph on the documents. The addressograph contained Patient A's name, date of birth, and medical record number. On May 31, 2013, at 9 a.m., an interview was conducted with the compliance officer. She stated, the patient contacted the hospital and she brought the paperwork back. The Registered Nurse that made the mistake took additional training regarding Health Insurance Portability and Accountability Act (HIPAA).
