While the VA Police staff were doing rounds on 06/24/12 a surgery schedule and a list of patients being reviewed at the Morbidity and Mortality (M&M) committee were found in the hallway outside of the ICU. There are 9 individuals with full SSN, name and surgery performed on the surgery list, 3 on the M&M committee list with name, last four digits of the SSN, and reason for review at M&M committee Update: 06/27/12:Nine Patients will be sent letters offering credit protection services.

The person responsible was educated on the proper disposal of PHI and PII
