Based on record review, the hospital failed to safeguard Patient 1's Protected Health Information (PHI) which resulted in an unauthorized person receiving his PHI.Findings:During the review of the facility Unusual Occurrence Report dated 5/21/13, the report indicated Radiology staff A was attempting to fax a report to an outside clinic. However, she faxed the information to an outside private business.Radiology Staff A was re-educated on patient privacy and completed training on faxing information.
