Issue:

An employee at the VAMC found a page of Veteran A's Progress Note in the snowbank alongside a driveway at the facility. It is not clear at this time if the note in question was given to the Veteran at the episode of care for education purposes or if VAMC staff is responsible for the loss. The Privacy Officer (PO) has attempted to contact the likely author of the Progress Note to determine who is responsible for the disclosure. Update: 01/28/13:The Resident has not responded to either email or paging. The Progress Note that was found is not a note in the Veteran A's medical record; however, it looks like it was copied and pasted into a different note. Based on this, it is not likely that the note was given to Veteran A for education purposes. The Veteran will be sent a letter offering credit protection services.

Outcome:

Supervisor of responsible employee was notified of this incident and is taking appropriate supervisory action.
