Issue:

This morning at about 9:30 AM, one of our employees found another employees W2 form in the Hardees Parking Lot. Inside the W2 form was a number of Veterans full name, full SSN and DOB. The employee turned the papers in to her supervisor at the VA immediately upon coming to work. Upon evaluation it was determined that the W2 belonged to a VA employee. Inside her W2 was her daily work sheet for 2/15 and a sticky note which had the patient names and other identifiers on it. Each Veteran was 1.) From different units in the Community Living Center (CLC). 2.) Purposely written down on a stick note 3.) All were known to be service connected in some way. Two additional Veterans information was found on the back of her assignment sheet with what appears to be income figures. There is a total of nine Veterans with full name, full SSN, and DOB and protected health information (PHI). All nine of the Veterans on her list were service connected. Police have been notified and are working on a report. The Information Security Officer (ISO) has been notified and is involved. The Privacy Officer (PO), ISO and Police will further investigate.

Outcome:

02/19/15: The Incident Resolution Service Team has determined that nine Veterans will be sent a letter offering credit protection services.
