Release of Information (ROI) mailed medical records on a deceased patient to life insurance company on January 4, 2012. On January 26, 2012 the alternate Privacy Officer (PO) received the medical records from the USPS with letter stating that the records had been found loose in the mail or was damaged in handling by the postal service. There are approximately 25 pages missing from the released medical records. Update: 01/27/12:This is a UPS delivery error. No fault of VA.
