Issue:
Employee A's separation notice of retirement was sent to Employee B in error. Employee B reported the error to our Human Resource Department and returned the form to our medical center. Employee B notified Employee A of the inconsistency (these 2 employees worked with each other for years and retired from federal government service in the same month). Update: 07/05/12:One employee will be sent a setter offering credit protection services due to full name and SSN being disclosed.
Outcome:
Human Resource staff were reminded on the importance of security and privacy safeguards.
