Issue:
VA employee/Veteran requested a SPAR from the Information Security Officer. On the SPAR he noticed two (2) employees accessed his medical record that is not involved in his care. The Privacy Officer has sent notice to the employees to ask why this employee records where accessed. Update: 04/08/13:The Veteran/employee will receive a letter offering credit protection services due to inappropriate access to his medical record.
Outcome:
Supervisor has been notified of the issue and action letters have been sent to supervisor for the two employee.
