Issue:

Last week, Veteran A contacted the Business Office (Murfreesboro Campus) to request an enrollment packet be mailed to him. Veteran A is not currently enrolled in TVHS.Yesterday, October 13, 2011, Veteran A received the enrollment packet in the mail and noted it also contained the completed enrollment information for Veteran B. Veteran A reported the incident to Privacy Officer (Nashville Campus) and returned Veteran Bs enrollment packet to the Business Office (Murfreesboro Campus). PO confirmed the packet has been returned. Veteran Bs Personal Identifiable Information (PII) included in the packet: Full name, full social security number, date of birth, address, home/cell phone numbers, military information, and financial information. Update: 10/14/11:Veteran B will be sent a letter offering credit protection services.

Outcome:

11/7/11 - Incident caused by human error. No specific employee identified. Business office staff has been reminded of the importance of checking documents for accuracy before mailing. Enrollment packet was returned to Business Office.
