Issue:
A phone call was received on October 20, 2011 informing a pharmacist that a Veteran who resides at the Wyoming Veterans Home in Buffalo would be on pass to Arvada, CO visiting relatives. It was requested that his medication be sent to an address in Arvada. A prescription was filled and mailed via UPS on October 21, 2011. The pharmacist did not document in CPRS the request or the name of the person requesting the refill. The package was signed for and received on October 24, 2011 at 10:22 am as confirmed through UPS tracking. The signature appears to have the same initials as the Veteran but is illegible. On November 3, 2011 a call was received from the Wyoming Veterans Home in Buffalo requesting the Veterans medication. The Veterans Home informed the Sheridan facility that the Veteran had never left the Veterans Home and did not have relatives in Arvada. A Physician was called to renew the medication and medication has been provided to the Veteran at the Veterans Home in Buffalo. The OIG has been notified and provided the address that the controlled substance was delivered to. OIG stated they would check the address. Update: 11/07/11:The Veteran will be sent a notification letter.
Outcome:
Sheridan VAMC is looking at processes, and have provided education to staff to ensure that documentation takes place in CPRS regarding the request and who it was requested by.
