Issue:
The Release of Information (ROI) Supervisor contacted the Privacy Officer (Nashville Campus) regarding the following incident: Veteran A contacted ROI Supervisor to report he received medical records in the mail which belong to Veteran B. The PO (Nashville Campus) contacted Veteran A to inquire as to how many records and/or what type of records were received. Veteran A did not want to discuss specifics. However, Veteran A agreed to return Veteran Bs medical records to TVHS. PO made arrangements to mail Veteran A an envelope in which to return the medical records.
Outcome:
05/31/13: Veteran B will be sent a letter offering credit protection services.
