Issue:

Based on interview and record review, the facility failed to correctly identify one patient (A) prior to placing a patient identification band on his wrist which resulted in a breach of protected patient information.Findings:During a review of the "Report of Unusual Occurrence" dated 8/21/13, the facility reported that Patient Access Services Representative 1 (PASR 1) placed the patient identification band for Patient B on the wrist of Patient A. The identification band included the following information: name, sex, date of birth, medical record number and account number.During an interview with the Privacy Officer, on 8/28/13 at 8:23 AM, she stated that PASR 1 did not verify the patient identifiers with Patient A prior to placing the identification band on his wrist which resulted in Patient A being banded with Patient B's information. She stated Patient B notified facility staff that he information on his band was not him.
