Issue:
Veteran A called to complain that an employee gave her protected health information to her insurance company without her signed authorization. The investigation revealed that the insurance agent called the physician's office to obtain information about the dates of service and treatment for a condition that the Veteran had made an insurance claim. The employee provided the information without the Veteran's authorization. The employee admitted to providing the information.
Outcome:
06/16/14: The Incident Resolution Service Team has determined that Veteran A will be sent a HIPAA notification letter due to Protected Health Information (PHI) being disclosed.
