Issue:
Based on interview and record review, the facility failed to prevent unauthorized access to a patient's PHI (Protected Health Information).Findings:On March 18, 2013, an Admitting Department staff member interchanged two patients (Patient A and Patient B) co-pay letters when placed in envelopes. On that same day, one of Patient A's family member called the Admitting Department to report being given Patient B's co-pay letter. Patient B was notified of the interchange of co-pay letters. The letters included, the patient's name,and medical record number.During an interview with the CO (Compliance Officer), she stated, "Two patients received each others co-pay forms. When mailed out, the co-pay letters were placed in the wrong envelopes. We found out when a patient's father called."
