Issue:

An employee reported leaving paperwork in a folder on top of his vehicle and forgot to remove it before driving off. After realizing the error, the employee went back to the location and picked up the paperwork but could not find a copy of a completed travel voucher form with the Veteran's name, address, and full SSN. The employee reported searching the area several time the next day and still was not able to locate the travel voucher form. This Privacy Office will investigate further. Update: 04/19/11:The Veteran will receive a letter offering credit protection services.

Outcome:

Corrective action plan includes: completing the required Authorization to Transport and Utilize VA Sensitive Information Outside Protected Environments form, social security number will be blocked out before transporting a copy of the payment voucher, all paperwork that contains VA sensitive information will be placed in an envelope that will be fastened, envelope will then be put in a carrying case and immediately placed inside the car. Employee involved has taken Privacy and Information Security training. Credit monitoring letter was mailed to the affected Veteran.
