Issue:

On 07/17/13, a Veteran reported the following incident to the Privacy Officer, Nashville Campus: In May, 2013, Veteran A received a routing slip in the mail from TVHS (York Campus). The routing slip had been mailed in a window envelope. Veteran A stated the envelope arrived torn and the routing slip was folded in such a manner the viewer could see his full name, full social security number, and address. Upon investigation, the Privacy Officer (PO) found: PO cannot confirm if the envelope was inadvertently torn by TVHS or the U.S. Postal Service. The presumption is the routing slip was sent to Veteran in lieu of an appointment letter. The envelope the Veteran provided the PO was not the original envelope. Veteran had thrown the original envelope in the trash. Instead, Veteran placed the routing slip in a discarded VA envelope he had at home and tore the envelope to demonstrate how his information had been exposed. The original envelope likely contained the routing symbol of the department which mailed the information. Without the original envelope, PO cannot confirm which area of the medical center mailed the information. In the future, Veteran will save the original envelope and immediately contact the PO if a similar incident occurs.

Outcome:

07/19/13: Veteran A will receive a letter offering credit protection services.
